Know what every pill in your cabinet is actually doing
Your cardiologist writes one prescription. Your primary care doctor writes another. A specialist adds a third, and the bottle from two years ago is still sitting on the shelf. Each prescriber sees a slice of the list. Nobody has read the whole thing start to finish.
Medication therapy management is that reading. You bring what you take to the counter in Culver City, and a pharmacist goes through all of it with you, including the things no doctor ever wrote down.
Everything on the table at once
Every bottle
Prescriptions, vitamins, supplements, and whatever you picked up off a shelf.
One sitting
The full list gets reviewed together instead of one refill at a time.
A written summary
You leave with what to keep, what to question, and what to raise with your prescriber.
Sean Rahimi, the pharmacist who owns the counter, holds a Medication Therapy Management certification and has been practicing for more than ten years.
What the pharmacist is looking for
Two drugs, one job
Separate prescribers can put you on two medications treating the same condition, and neither one has any idea about the other.
Combinations that work against each other
Some pairs blunt each other, some stack the same side effect, and the odds of hitting one climb with every bottle added.
A dose that stopped fitting
An amount set years ago can be wrong today if your weight, your kidney function, or the rest of your list has shifted since.
Doses that never get taken
Skipped pills surface fast in a review, and the fix is often as small as moving one dose to a different hour or switching to packaged strips.
Bring these with you
- Every bottle in the house, including the ones you quit taking.
- Vitamins, supplements, and anything herbal.
- Your insurance card, so a Medicare Part D assessment can run at the same visit.
- The name of whoever prescribed each medication.
- Reviews happen Monday through Friday, 9:30 AM to 5 PM. The counter is closed both weekend days.
Ask for a medication review and we will hold the time for you.
