Every Medication Sorted, Checked, and Brought to the Door
Nine prescriptions from four prescribers is a logistics problem before it is a medical one. Refill dates land on different days. One bottle empties on a Saturday. Somebody has to get in a car for a single pack of pills.
Long term care here covers both settings: residents in assisted living, and older adults still in their own homes in Culver City with family or an aide keeping track. The work is the same either way. One list, one schedule, one pharmacist reading it.
From First Call to Front Door
The Intake
We move the prescriptions over and build one profile for the patient.
The Pack
Doses go into Dispill or multidose packs marked by day and time.
The Drop
Home delivery brings them to the residence or the facility.
Culver City Senior Housing sits in the same retail center as the store, and several more senior living communities are within walking distance.
What the Pharmacist Catches
Two drugs doing one job
A medication therapy management review reads the entire list at once, which is how duplicates and risky pairings from separate prescribers surface.
Refill dates drifting apart
Med Sync pulls every prescription onto the same monthly date, so a caregiver makes one call instead of six.
Coverage that stalls a dose
Prior authorization paperwork and Medicare Part D assessments get handled at this counter rather than left to the family.
The same pharmacist each time
Sean Rahimi owns the pharmacy and serves as pharmacist in charge, so the person reviewing the list this month is the one who reviewed it last month.
What we take on

- Sourcing medications that are hard to find.
What we need from you

- The insurance card, or a question about cash pricing if there is no plan.
- The name of whoever manages the pills day to day.
- Requests placed Monday through Friday, 9:30 AM to 5:00 PM, since the store is closed on weekends.
Tell us what the patient takes and where they live, and we will set the first delivery.
